Complete Oil Sample Registration Form
Please complete the form below as detailed as possible. To get the best results, the right information must be collected prior to laboratory processing. The information provided in the form will be used to evaluate each sample, so accurate information is critical for obtaining accurate results. A confirmation email will be sent to the email address used to complete the form.

Submit oil sample
Package up your sample bottle using the plastic bag and the pre-paid USPS Priority Mailbox. The box is already addressed, so you only need to place the sample in the box, seal, and ship. The box can be dropped off at any USPS post office or handed to your local mail carrier. Depending on the service level of the kit, the results will be emailed within that timeframe.
Please Note: A sample can only be submitted using the unique ID number sample bottle supplied by SPEEDiagnostix.
FIRST TIME USERS: When you click the “submit” button and it changes colors, scroll back up the page and complete the reCaptcha image challenge (to prove you are a human). After that, scroll back down and hit submit again. That will complete the submission process. You will receive an email with your submission form into to verify the submission was entered.
